We are so excited to meet you and your beloved pet. Our veterinarians want the process of beginning your pet’s health care at our animal hospital to be smooth and easy. For those of you with anxious pets (or maybe even anxious dispositions themselves) we’ve given you a list of what to expect for when you first visit our animal hospital. You’ll also find several forms and tools below, which we’ve provided in order to do just that.

What to Expect from Your First Visit

When you come to your first appointment, you can find our pet clinic located conveniently just off the road. We keep our parking lot and large animal clinic facility clean and available for clients, old and new, as any professional medical practice should be.

From your first visit to Oak Forest Veterinary Hospital, we’ll do our best to help you feel at home. First, you can expect to wait in our living room (otherwise known as the lobby) while we get your exam room ready for you. While you wait, we have a coffee and tea bar available, so please take advantage of our hospitality. We’ll see you soon.

Every new client gets a new client folder with information on pet insurance, pet boarding and who they are. Each new client additionally receives an extended appointment time.

At Oak Forest, you can rely on the fact that we always provide a treatment plan prior to treatment so no one is walking into any appointment blind.

And for anxious pets, our team has some yummy tricks up our sleeves to help keep pets happily distracted while we ensure they receive the best possible veterinary care. We think of the little things, from ensuring we have the sharpest needles to help prevent vaccinations from hurting, to providing diffusers with pheromones for the cats, and even little pieces of cheese here and there for nervous dogs; we know how to help put your pet at ease.

Pet health care issues are not always predictable, so we make it as easy as possible for you to let our veterinarians do their job of pet health care delivery while you do your job at the office or at home. Complete and print this form out and hand it in when you drop your pet off on your way to work.

Pet health emergencies require a little more paperwork than regular visits to the veterinary clinic. We have all necessary forms available online for you to fill out before surgery day arrives.

In the interest of you and your pet’s well being and peace of mind, as well as that of every member of the Oak Forest Veterinary Hospital family, we establish certain requirements for bringing your pet in for our wonderful pet boarding services.
