In order to maintain a comfortable environment for everyone staying with us, all pets boarding will be administered a Capstar tablet
at an additional cost of $6.94 per pet. Capstar kills all live fleas within 30 minutes and continues to treat for 2 days. If fleas are found
on your pet during his/her stay, we will also treat an appropriate longer-lasting flea product.
(Please be aware that if your pet is on a
special diet and you did not bring it with your pet, you will be charged for the special food while boarding.) Due to health and sanitary
concerns, we cannot accept and feed a raw diet.
Please list any medications; including the dose and frequency that your pet needs to receive while boarding at OFVH.
If you choose to leave bedding, bowls, or toys for your pet, you do so at your own risk. We have plenty of these items for your pet’s use and your own items may be
misplaced or damaged.
Oak Forest Veterinary Hospital (the "Facility"), as is allowed by Texas law, is not equipped with an on-site fire suppression sprinkler system and does not employ on-
site personnel during non-business hours. All animals boarded at the Facility will be left unattended during those times.
By signing below, you are authorizing Oak Forest Veterinary Hospital to perform any necessary medical care while <animal> is boarding in the event he or she should
become ill and assume responsibility for charges incurred for the care of my pet(s). Sometimes boarding pets become stressed and do not want to eat. If <animal> does
not eat for us, we may add wet food to your pet’s normal diet. These charges will be added to your boarding fees. Any pet left longer than 30 days without
communication from you, the owner, will be considered abandoned.
I hereby grant Oak Forest Veterinary Hospital permission to take photographs of myself and/or my pet, and to publish those photographs for any lawful purpose,
including, but not limited to, their website, social media accounts, and promotional materials, either digital or in print, in perpetuity. I also grant permission to use my
name and/or my pet’s name and waive any rights of privacy or compensation associated with the use of my or my pet’s image(s) and name(s) for the personal or
commercial purposes outlined above.
By signing below I acknowledge that I have received and read the Facility’s notice on its lack of fire suppression and overnight staffing. I agree to release the Facility,
its owners, employees and agents from any and all liabilities, claims, or expenses arising from my pet’s boarding stay, including but not limited to injuries, illnesses, or
death. I have read and understood the statements outlined in this consent form and release. I voluntarily consent to my pet staying at the Facility and acknowledge that I
am solely responsible for any consequences that might arise during their stay.
